Grapefruit and Medication: Pharmaceuticals Expert Advises Caution

As part of a healthy diet, consuming fruits and vegetables is encouraged. However, individuals taking specific medications must be cautious of certain fruits due to their interference with the efficacy of the medication. In England, a significant number of individuals are prescribed statins or ezetimibe to manage cholesterol levels, with an estimated 7-8 million adults in the UK taking statins. This widespread use makes it crucial to understand potential interactions between medications and common foods.

Pharmacy Practice senior lecturer Dipa Kamdar highlighted the need for careful consideration when making dietary changes, especially for those on prescription medication. She explained that grapefruit juice contains compounds called furanocoumarins, which can inhibit the enzymes used by the liver to break down certain prescription drugs. This interference can lead to the accumulation of drugs in the body, potentially causing adverse effects.

Specifically, medications such as ciclosporin, commonly used in organ transplant rejection prevention and for the management of conditions like rheumatoid arthritis and psoriasis, can lead to serious side effects if their levels in the body are heightened due to grapefruit juice consumption. Additionally, statins used for cholesterol management and other drugs like amlodipine and sildenafil could also be affected by the interaction with grapefruit juice, potentially increasing the risk of side effects.

Health experts from the NHS and the British Heart Foundation advise individuals taking certain statins to avoid or limit their intake of grapefruit and grapefruit juice to prevent elevated drug levels and associated risks. It is crucial for individuals on these medications to consult their doctor or pharmacist before consuming grapefruit products, and in some cases, total avoidance may be recommended to ensure the effectiveness and safety of the prescribed medication.
